Applying for the CGFNS Thailand ISPN Program

To apply for the CGFNS Thailand ISPN Program, you need to be a citizen of Thailand, have graduated from a government-approved school and have your status as a first-level, general nurse verified.*

*If you do not meet these requirements and send us the application and fees, your money will not be refunded.

How to apply

  1. You can complete the pdf application form online by clicking the link below. The application has fields you can click into and type your information. You can also download the application and if you have Adobe Reader, you can fill out the fields offline.

    2. Mail the main application (pages 1 to 5), the Photo Identification Form (page 7), the Authorization to Release Information form (page 8) and the Credit Card Payment Form (page 9) together with requested photo and documents and the fee to ISPN Thailand, c/o CGFNS International, 3600 Market Street, Suite 400, Philadelphia, PA 19104.

    Please complete the top portion of the Request for Validation of License/Registration/Diploma form (page 6) and send to your licensing authority.

  2. You can print the application and forms and complete them by hand. See the instructions above for where the application and each form is sent.

Once CGFNS receives the completed application, the completed forms (including those from your licensing authorities), all requested documentation and the fees, they will be reviewed for approval. Once approved, you will be sent an Authorization to Test notification with instructions.

Fees

The fee for the CGFNS Thailand ISPN Program is US$300.

CGFNS is offering the special price of US$200, a US$100 discount, to the first 100 Thai nurses who apply for the ISPN Program in Thailand and are eligible to take the November 2011 CGFNS Qualifying Exam®.
