Document questions
Please only send us photocopies of your secondary school diploma (or other documents, such as marriage certificates to document a name change). However, regarding post-secondary (tertiary) education academic records/transcripts and license/registration/diploma, CGFNS only accepts original documents directly from the issuing authorities. In addition, since we verify authenticity, we reserve the right to request original documents.
If you no longer have your original diploma or other documents, you will need to contact the organization that issued them and ask them to send an official letter verifying your documents directly to us (3600 Market Street, Suite 400, Philadelphia, PA 19104-2651 USA). We accept external examination results only if provided by the examination board.
No. Once documents are received, they become the property of CGFNS. Please do not send your original documents to us.
Documents that must come directly from your school’s officials, licensing authorities or other source agencies can be translated by them and sent directly to us in the signed, stamped and sealed envelope.
Documents that you are permitted to send (such as marriage certificates, secondary school diplomas) can be translated by you or anyone you choose. These translations must contain the following, called the Certificate of Accuracy, typed or written at the end of the translation. The translator must sign the translation.

This is to certify that this is a true and correct English translation of the attached photocopy of the original [insert name of document] of [insert applicant name].
CGFNS can also provide translations for the fee listed at http://www.cgfns.org/sections/apply/fees.shtml#5.
Credentials Evaluation Service
Check with your CES report recipients to verify the type and specific credentials they wish to see in it. Academic transcripts and license/registration/diploma validation need to be sent directly from the issuing authorities to us. Refer to the resources on the Credentials Evaluation Service page at www.cgfns.org/sections/programs/ces/default.shtml.
Yes and no. The documentation required for the Certification Program differs from that required by the Credentials Evaluation Service. It may be necessary for you to request a full academic transcript from your schools, listing each post-secondary (tertiary) health care related coursework you completed, since a full transcript may not have been provided by the school under the Credentials Evaluation Service requirements. If you are uncertain about what we have for you on file and what further documents are required, contact our CGFNS Customer Care Center for more information.
No. New York State requires that CGFNS directly make the requests to your schools and licensing authorities, instead of you making the request.
No. CGFNS will automatically include a certificate statement as part of your report to the New York State Education Department, if applicable.
